How to print your USAV Membership Card

1) Click "HERE" to access the membership registration website
2) Login with your username and password
3) To the left of the screen you will see an icon "Print Membership Card"

If you do not have the option to print a membership card call the CEVA office at 503-644-7468



How to register for a USAV day pass

1) Click "HERE" to print the individual membership registration form.
2) Bring the completed two page form and an additional $10 to the event.
